GET
HELP!
It
is the policy of the Enuresis Advisory Service to recommend that
patients find and accept help from a Professional Health Carer.
It has been our experience, and that of others, that patients who
accept help and informed guidance from a Health Care Professional
have a much higher success rate in managing an enuretic problem
than those who do not seek support.

Professionals who manage and treat enuresis.
Do
have your child examined by your G.P for possible physical problems
and have the urine examined and checked in a lab for infections.

"Reward
Systems in Behaviour Management"
An
article by Paediatrician Dr David Cutting of Lilydale in Victoria
Achievement
of dryness and appraisal from loved ones are the greatest reward.
More tangible rewards are a useful part of management programs for
enuresis. They act as enhancements rather than sole strategies.
Principles:
-
- They
provide concrete evidence of achievement.
- Must
be given GENEROUSLY not stingy or over board.
- CANNOT
be withdrawn.
- NO
bad marks.
- Above
all must be ACHIEVABLE.
Start
Simply: -
Do
not set the child up to fail by making it too hard to achieve soon
(eg. dryness every night when wet every night may be too distant
a target).
Examples
of good targets:
- Following
the program instructions.
- Cleaning
the alarm.
- Reduction
in size of wet patch.
- Going
to the toilet on waking.
- More
urine passed in the toilet on waking.
- Fewer
wet episodes.
- More
dry nights.
As
the parent is the judge it should always be possible to find a positive
outcome.
Simple
Rewards-
- Star
charts.
- Join
the dots ("digitise" a desired reward into dots/numbers
and join two dots with each "success"). On completion provide
that reward.
Rewards
might be:
- Choosing
the day’s clothes for the day’s activities.
- Special
food for school lunches.
- Ultimate
reward - extra TV.
Rewards
can be combined with progress/star charts where several stars are
needed to "cash in" to achieve a particular award, eg.
if 5 stars are achieved for following instructions an extra TV program
is allowed.
Beware
(things NOT to do or to allow): -
Bribery "if
you do this I will...".
Blackmail
"if you don’t give me a reward I will....".
Stagnation
(so continually change the award type).
Sibling
grumbles "he gets rewards for being dry, I am dry and get nothing".
Secrets
"I know it is difficult but if you help I will slip you something
on the side".
Rewards
provide valuable additions to behaviour management and to alarms.
They must be achievable. They must not replace the focus of the
program, ie. achieving dryness. Rewards should provide fun and enjoyment
for both child and parents.

HELP
FOR DEEP SLEEPERS
Recent
research suggests that some children really do need extra help to
wake up. There are a number of strategies to help the heavy sleeper
including delaying urination until later in the night - when the
child is more likely to wake to a loud noise.
The
use of Minirin® (Desmopressin®) in a dose less than results
in complete dryness, can be a valuable ally in delaying urination.
Minirin® combined with an alarm may move wetting to a time when
the child will be responsive enough to respond.
Some
children need more daytime drinks to increase bladder emptying volume,
and some need medication to delay bladder emptying.
Another
strategy that may be tried is to use the Malem® Bed Mat alarm
with three Malem® Alarms attached.
This
is noisy enough to wake most heavy sleepers - and the rest of the
household too!
